I’m delighted to be working with a small domiciliary care provider in Northampton who are now looking to welcome a Care Coordinator to their friendly team.

What’s on offer:
£22,000-£24,000
28 days annual leave
Additional on-call pay (receiving calls only)
Free parking
Care Coordinator Responsibilities:
Communicating regularly and professionally with clients and their families to ensure their care needs are being met and any concerns are promptly addressed.
Liaising with pharmacies to ensure timely and accurate delivery of medication for service users.
Supporting the Registered Manager with the completion of client assessments and risk assessments to maintain safe and person-centred care planning.
Managing and scheduling staff rotas to ensure consistent and effective care delivery across the service.
Responding to last-minute care changes or emergencies to minimise disruption to service users.
Building strong relationships with care staff to promote high standards of performance and job satisfaction.
Care Coordinator Requirements:
Experience working in a Care Coordinator role for a minimum of 12 months.
UK Driving Licence and use of own car.
