We are looking to recruit a Social Care Team Leader on a temporary basis, to join Trafford in delivering high quality person centred care and support across supported living homes in Trafford.
Working pattern is full time hours.

Responsibilities and Duties:

* To lead manage and supervise a team of care workers to ensure they provide high quality person centred care and support.
* Ensure that regular house audits and spot checks are completed as required.
Manage and support care workers so they can perform their roles safely and to the best of their ability. This will include ensuring appropriate training and opportunities for personal development.
* Ensure that person centred care plans are developed that outline what care and support individuals need.
* Acting as a positive role model to colleagues you will promote a safe, well cared for living and work place for people
* Lead team meetings and undertake inductions, supervisions and appraisals
* Working in partnership with other professionals you will ensure individuals get the support they need.

Key duties:

* To supervise Support Workers on a day-to-day basis, ensuring that services provided by the team are of a consistently high quality, ensuring continuity of person centred care and support and adhering to the guidelines of best practice.

* To allocate tasks to Support Workers on a daily basis and to effectively co-coordinate all available resources to ensure the safe, efficient and effective running of the service.

* To ensure that safeguarding incidents are reported in accordance with those policies, practices and procedures of the Local Authority and Regulatory Authorities including the Care Quality Commission.

* To support the Registered Manager to monitor staff performance. To identify training and development needs of the team by carrying out skills performance assessments and supervisions and PDRs if required.

* To support the Registered Manager to ensure that safe working practices are adhered to at all times, promoting a safe and healthy working environment for all staff and Service Users, this includes carrying out and reviewing risk assessments within required timescales.

* To ensure support and care plans are reviewed and updated as necessary within the principles of person-centred care planning and within required timescales.

* To liaise and communicate effectively with other professionals, Service Users and their families / carers / representatives / advocates as appropriate; so as to ensure a smooth transition through the service and achieve best possible outcomes for the individual.

* To ensure that Service Users are fully included in any decisions made in relation to the service they receive.

* To work effectively alongside colleagues within the department to ensure the safe, effective and efficient delivery of a flexible, high quality, borough-wide service.

* To assist and support the Registered Manager to monitor service quality by completing Service User Reviews and distributing / assisting with Service User Questionnaires as necessary.

* To make accurate records of information as directed by the Registered Manager and to submit information in the required format and within required timescales.

* To undertake any other duties that is commensurate with the grade.

All duties must be carried out to comply with all relevant legislation and statutory requirements, National Minimum Care Standards, General Social Care Council Codes of Practice and Good practice guidelines and standards within National Vocational Qualifications.

* Enhanced DBS Check
* Level 3 Qualification in care, or equivalent
