At Leaders in Care, we are dedicated to supporting individuals as they transition from hospital to home or community settings. We work closely with hospitals, community health services, and social care teams to ensure seamless and effective care coordination.

The Senior Social Worker role involves:

Supporting and managing the hospital discharge pathway service.
Providing expert advice on complex cases.
Helping the team deliver the best possible outcomes for patients.
Driving service improvement and efficiency.
