We are looking for an experienced Community Nurse to join a nationally recognised specialist provider, as part of their exciting 2020 growth plans.

Working Monday to Friday / 9am-5pm hours, in the Kent area you will have complete autonomy to plan your own working week. You will typically see between 5 and 7 patients a day, meaning you will have ample time to build strong relationships with your patients and their loved ones.

This company provide care to over 200,000 patients per year across 75 home care and over 400 chemotherapy care packages. With over one million interactions with varied NHS and private patient populations you will be joining a team of 450 specialist highly trained Nurses as well as a 24/7 patient advice line and a cold chain logistics operation.

Key responsibilities for a Community Nurse include:

Delivering one to one care in line with the patients' needs
Building lasting relationships with patients, stakeholders and patient support networks to aide and support patients treatment and recovery.
Carry out clinical reviews with clients
Attend client MDT's
Support discharge home from hospital
Make referrals where appropriate
Observe carers and take them through the competency frameworkThere is an excellent selection of benefits on offer for any successful Community Nurse as per below:

RGN Qualification
Valid NMC Pin Number
Strong clinical skills
Experience in the community or ward setting
