General Summary of Duties: Responsible to carry out the tasks of personal care as designated by #a written care plan provided by A Caring Hand, LLC. Assists in maintaining safe environment for clients, employees and visitors. Primary population is geriatric. Supervision Received: Reports directly to the Client Service Manager Supervision Exercised: None Typical Physical Demands: Requires full range of body motion including moving and lifting patients, manual and finger dexterity, and eye-hand coordination.# Requires standing, walking, ability to ascend and descend stairs.# Requires vision and hearing corrected to a normal range.# Occasionally lifts and carries or moves items, 50 pounds or more.# Requires working under stressful conditions. Typical Work Conditions: Work is performed in varied home settings.# Some exposure to communicable diseases, body fluids, toxic substances and medical preparations. Essential Duties: Assisting the client with personal care and activities of daily living that# include bathing, oral hygiene, toileting, feeding, dressing, exercises, ambulation, transfers, and medications that are ordinarily self-administered. While following safety protocols. Maintains skin integrity by following the plan of care regarding of turning and correct positioning of the client Providing normal household services essential to client care at home, including shopping, meal preparation, laundry, and light housekeeping duties. Providing companionship and stimulation for the client including reading, walks, etc.; and accompanying the client to doctors or other appointments. Observing and reporting changes in client#s condition to Client Service Manager.# Adheres to all agency policies and procedures including time sheet and activity sheet documentation, patient confidentiality, good attendance and works within boundaries of job responsibilities. Communicates appropriately with clients and families.# Communicates at client#s level of understanding. Adheres to confidentiality policy regarding client scheduling, work assignments, agency-related work issues, personal issues, and avoids controversial topics. Meets in-service requirements. #Adheres to Agency HIPAA Privacy Plan as per written agreement. # Ancillary Duties: Performs other duties as necessary to support the Agency as assigned by the Client Service Manager. # HIPAA and Privacy:# Properly direct any complaints and/or grievances as indicated per A Caring Hand policy.# Comply with HIPAA regulations, specifically the Privacy and Security Rules.# Protect personal health information (PHI), electronic personal health information (EPHI) and financial information by limiting the use and disclosure of health information and financial information to the minimum amount necessary with only those who must know how to get the job done.# Follow the agency#s specific policies and procedures to protect health information.# Help one another to ensure confidentiality at all times. # Knowledge, Skills and Requirements: Ability to read, write and speak English.# Ability to comprehend written and oral instructions in English.# Ability to drive a car, possession of a current driver#s license, and access to a car.# Must show proof of automobile insurance with State of Connecticut minimum liability coverage.# Flexibility; must be punctual and dependable.# Ability to get along with people, a sympathetic attitude and ability to adjust to varying complex home situations; ability to motivate; ability to work well with others.# Desire and motivation to grow in knowledge and to attend the required# #in-service education.# Must have excellent communication skills and ability to follow directions, both written and verbal, to adhere to Agency Personnel Policies and Dress Code, and to maintain confidentiality. # # Education: High school diploma or GED preferred # Experience: Previous experience as CNA/HHA preferred # Certificate/Registration: Certificate to practice as a Home Health Aide/ Personal Care Attendant #in the State of Connecticut or is enrolled in or has completed a Basic Approved Training Program as defined by Licensure Regulations or evidence of meeting the exemption criteria as defined by the Licensure Regulations. Or CNA with Certification with the Connecticut State Nurse Aide Registry. # Alternative to Minimum Qualifications:# None
