Job Summary and Qualifications



 



Collaborates with colleagues in Care Management department, service line, and interdisciplinary care team to insure quality driven, best practice across the continuum of hospitalization by utilizing the eight essential activities of case management in direct patient/client contact: assessment, planning, implementation, coordination, monitoring, evaluation, outcomes and cost containment. Maximizes positive financial outcomes for designated patient populations by intervening at key points in the disease process with post discharge follow-up phone calls and crucial communication with care providers to promote health compliance and prevent readmissions.Facilitates SNF infection prevention and control activities with the primary goal of reducing risk of nosocomial infections. Infection prevention and control activities include perform continued collection, review, and analysis of nosocomial infection data; conduct benchmarking analysis of nosocomial infection data, prevention procedures, policies, and initiatives with national standards and best practices; identify improvement opportunities, provide recommendations to clinical areas, and facilitate implementation of best practices across the hospital system; provide patients/staff with relevant and current infection prevention education. This position has regional responsibility.



Required Education: Associates Degree Nursing



Preferred Education: BSNRequired License/



Certifications: Current licensure as Registered Nurse with the North Carolina Board of Nursing or a compact state as defined by North Carolina Board of Nursing. Current BCLS. Infection Prevention or within 2 years of hire.



Required Experience: One year recent clinical nursing experience in relevant practice area or in case management.



Preferred Experience: Two years recent clinical nursing experience in relevant practice area. One year managerial or Case Management experience.



 



Highlands-Cashiers Hospital is a community hospital serving Macon, Jackson and the surrounding counties. Located between the towns of Highlands and Cashiers, the hospital offers 24/7 emergency care, acute inpatient care, rehabilitation, as well as long-term care through Eckerd Living Center. Ambulatory services are also available, including therapy services and primary care. Highlands-Cashiers Hospital has more than 24 beds for acute care services and 80+ beds in the Eckerd Living Center. Highlands-Cashiers Hospital is a member of Mission Health, an operating division of HCA Healthcare.
